If the school needs to administer long-term ‘as required’ prescribed medication please complete this form.

If your child is taking medication by themselves, please complete this form. We must have a record of any medication a pupil is taking whilst at school.

If your child has short-term healthcare plan not requiring medication in school, please complete this form.
